Pandemonium is the chaotically aligned Outer Plane of cold winds and madness.
About
Pandemonium is a cavernous plane, similar to The Underdark. Unlike The Underdark, Pandemonium's other defining feature is a never ending squall of psychic wind. It is said that the winds of Pandemonium strip the humanity from the minds of its victims.
Entrance to Pandemonium can be found via the river Styx, or the Infinite Staircase.
Pandemonium was also on the list of "possible convergence planes", noted for having the possibility of cross contamination with eldritch powers of the Underdark Old Ones.

Notable Locations
The Madhouse
The largest civilized population in Pandemonium, the Madhouse is a small town carved out by members of The Bleak Cabal. The city started as a singular tavern, but as it attracted visitors, other structures were built outward, protected by a dozen layers of protective walls, shielding the inhabitants from the worst of the winds.
Winter's Hall
Winter's Hall was a floating ice citadel located in one of the coldest caverns of Pandemonium. Khala, the original aspect of Winter, was the queen of the hall. It was from there that she and her sister ruled over the original Unseelie Court.
During The War of Winter, during which the citadel was shattered by The Raven Queen and her armies.
Long after it's destruction, Winter's Hall was the occasional residence of Loki.
